Parish Registration Form	Families new to the parish are asked to register with a priest in the sacristy after Sunday Mass or by appointment in the Rectory. This form may be downloaded, completed, and presented to the priest. Parish Registration Form
Request for Payment or Reimbursement	Parish organizations and individual parishioners may use this form to request a parish check to pay for or reimburse expenses made for a parish ministry or activity. All documentation should be attached to this request: e.g. invoices, receipts, etc. The completed form is submitted to the Rectory for approval and payment. Request for Payment or Reimbursement Form
Permission and Hold Harmless Forms	For participation in "high risk" sporting or recreational activities, the Archdiocese of Philadelphia requires parishes to: ensure disclosure of the high-risk associated with the activity to parents or legal guardians; have the parents or legal guardians acknowledge the assumption of the risk inherent in the activity; require the consent of the parents or legal guardians. Examples of "high risk" activities are skiing, snow tubing, white-water rafting, rock climbing, etc. The limited role of the parish in these activities includes only publicizing, arranging for group transportation, and/or arranging for group discounts.
